California’s advance directive is called the Advance Health Care Directive (AHCD) and comes in several versions:
- The Easy Form is an easy-to-read, easy-to-understand illustrated version of the AHCD for people with limited reading ability, with text in both Chinese and English.
- The Standard Form does not contain illustrations.
- The form can also be filled out online.
