The Importance of AHCD and POLST

CACCC suggests that it is a responsible practice to prepare for the medical care in advance, so that the designated person can follow the instructions given by the patient when he/she is conscious, thus avoiding the regret of family members who have different views on the matter. An Advance Health Care Directive (AHCD) applies to everyone over the age of 18. The Physician Orders for Life-Sustaining Treatment (POLST) applies to people who are seriously ill.
